Dr. Cohen does not participate in insurance plans.  Dr. Cohen will provide a receipt at the time of service that patients can submit to their insurance company for out of network reimbursement.
 
All medical questions and concerns should be addressed with a phone call or office appointment. 

Patients must be seen a minimum of every 3 months to ensure standard of care.

Payment is due at the time of service with cash, check, or credit card.

Refill requests should be made 3 days in advance to allow for enough time to fill prescriptions.

Cancellations made within 24 hours of the time of service will result in a charge for the appointment.
